Young Adult Questionnaire – Bridge to Independence Program (Age 19 – 20)

sending message

Thank you for responding to this questionnaire. Your input provides valuable insight on how the Bridge to Independence program is working, and suggestions on what needs improved. Your input also tells us how you are benefiting from the program. We collect data and provide a report to the Legislature (no names of young adults are used). We also by law provide a brief report to the court. You and your Independence Coordinator will receive a copy of the report.

Below is a list of broad topic areas that many people who have aged out of foster care need assistance with. These are likely topic areas you have discussed with your IC, and for some or all of them, you and your IC may have set specific goals to work on during your time in the b2i program. For each topic area:

  • Tell us if you have a goal related to that topic.
  • And if you do have a goal, tell us what it is, what you, your IC, and/or others are doing to help you achieve your goal, and how well it's going or if you are running into any problems along the way.
